B). Establecer los controles necesarios a fin de evitar que terceras personas no autorizadas puedan operar el servicio, sin perjuicio de la responsabilidad que asume el USUARIO por dichas operaciones, las cuales son equivalentes a las operaciones comerciales realizada hoy por otros medios. C). Seguir las recomendaciones en cuanto a forma de operar y seguridades del sistema. D) El USUARIO deberá guardar confidencialidad sobre la forma, manejo, servicios, facilidades, operación y funcionalidad.

Con la lectura y aceptación de esta información, usted y la empresa que representa, declaran conocer y se obligan a acatar las disposiciones aplicables a los proveedores de la Organización Carvajal allí dispuestas. El cumplimiento de estas políticas y normas es condición esencial para iniciar y mantener la relación comercial con cualquier empresa de la Organización Carvajal por lo que su incumplimiento puede dar lugar a la terminación de la relación contractual. La Organización Carvajal podrá modificar en cualquier momento dichas políticas, manuales y procedimientos, cuando sea requerido por la ley o con el fin de ajustar los mismos estándares éticos..

DECLARACIÓN DE COMPROMISO COMO PROVEEDOR DE CARVAJAL PULPA Y PAPEL

De acuerdo a la condición del servicio/bien prestado por EL PROVEEDOR dentro de las instalaciones de CARVAJAL PULPA Y PAPEL Y CARVAJAL PULPA Y PAPEL SAS ZONA FRANCA PERMANENTE ESPECIAL, el representante legal se compromete a cumplir con los parágrafos que apliquen:

1.PARAGRAFO I: CUMPLIMIENTO DE REQUISITOS LEGALES APLICABLES A SU ACTIVIDAD:

EL PROVEEDOR es el único responsable ante CARVAJAL PULPA Y PAPEL Y CARVAJAL PULPA Y PAPEL SAS ZONA FRANCA PERMANENTE ESPECIAL SAS ZONA FRANCA PERMANENTE ESPECIAL y ante las autoridades internacionales, nacionales y locales por el cumplimiento de los requisitos y obligaciones de acuerdo a la naturaleza de su actividad económica. EL PROVEEDOR acepta dar cumplimiento a normatividad legal vigente y a los requisitos exigidos por la ley de acuerdo a la actividad realizada por el PROVEEDOR y reconoce tener disposición para las inspecciones, visitas, solicitud de documentación pertinente, con el fin de que los funcionarios o delegados de CARVAJAL PULPA Y PAPEL Y CARVAJAL PULPA Y PAPEL SAS ZONA FRANCA PERMANENTE ESPECIAL puedan verificar su cumplimiento de acuerdo a la normatividad aplicable siempre que sea requerida. Las verificaciones pueden realizarse dentro de las instalaciones de CARVAJAL PULPA Y PAPEL Y CARVAJAL PULPA Y PAPEL SAS ZONA FRANCA PERMANENTE ESPECIAL y en su empresa. El PROVEEDOR es el único responsable por garantizar que toda la información y documentación que presenta es legible, completa y exacta brindando la confianza para CARVAJAL PULPA Y PAPEL Y CARVAJAL PULPA Y PAPEL SAS ZONA FRANCA PERMANENTE ESPECIAL sobre la actividad realizada.

2.PARAGRAFO II: OBLIGACIONES DEL PROVEEDOR Y/O CONTRATISTA EN MATERIA DE SEGURIDAD, SALUD Y MEDIO AMBIENTE:

EL PROVEEDOR Y/O CONTRATISTA es el responsable exclusivo por cualquier incumplimiento en Seguridad y Salud en relación con sus trabajadores, comunidades vecinas, autoridades gubernamentales, sus Subcontratistas y el medio ambiente. EL PROVEEDOR Y/O CONTRATISTA se obliga a acatar la normatividad vigente Nacional y de CARVAJAL PULPA Y PAPEL Y CARVAJAL PULPA Y PAPEL SAS ZONA FRANCA PERMANENTE ESPECIAL, en seguridad, salud ocupacional y medio ambiente, dentro de las instalaciones de la misma, así como desarrollar y asegurar para el Contrato los siguientes elementos mínimos de gestión:

1. RECURSOS: El PROVEEDOR Y/O CONTRATISTA debe realizar la planeación y seguimiento a los recursos económicos y físicos así como al talento humano requeridos para la ejecución del Contrato y el desarrollo del Plan Seguridad Industrial y Salud Ocupacional. El CONTRATISTA Y/O PROVEEDOR es responsable de disponer de los recursos necesarios para diseñar e implementar un sistema de gestión de seguridad y salud en el trabajo cumpliendo las disposiciones legales vigentes y dar cumplimiento al Plan específico del Contrato, así como de garantizar la integridad de las personas, el medio ambiente y la infraestructura operativa, durante la ejecución del contrato. Sin embargo, CARVAJAL PULPA Y PAPEL Y CARVAJAL PULPA Y PAPEL SAS ZONA FRANCA PERMANENTE ESPECIAL establece y requiere como mínimo el equipo de trabajo de acuerdo a lo siguiente: Contratistas por obra*: Se requieren Vigías de seguridad – suministrado por proyecto / servicio. Cantidad de Trabajadores Simultáneamente Cantidad de Vigías 1 – 50 1 51 – 100 2 Más de 100 3 por cada 100 trabajadores * Esto no aplica cuando se suministra personal por hora hombre Contratista por Outsourcing: Se requiere un coordinador de Seguridad Industrial, Salud Ocupacional y Medio Ambiente, tiempo parcial o total de acuerdo al riesgo.

Perfil del Coordinador de Seguridad y Salud: •Profesional de cualquier especialidad con experiencia laboral mínima de 4 años en Seguridad Industrial Ejecutando actividades de planeación, asesoramiento, dirección y/o aseguramiento de planes de Seguridad Industrial. Debe tener Licencia en Salud Ocupacional. •Tecnólogo en higiene y seguridad industrial o en salud ocupacional con experiencia laboral mínima de 5 años en Seguridad Industrial ejecutando actividades de planeación, asesoramiento, dirección y/o aseguramiento de planes Seguridad Industrial •Tecnólogo en otra especialidad. Deberla acreditar una experiencia mínima de 6 años en Seguridad Industrial ejecutando actividades de planeación, asesoramiento, dirección y/o aseguramiento de planes de Seguridad Industrial. Perfil del Vigía de Seguridad : •Profesional en Salud Ocupacional con experiencia laboral mínima de dos (2) años en Seguridad Industrial •Tecnólogo / Técnico en higiene y seguridad industrial. Deberá acreditar una experiencia mínima de tres (3) años en Seguridad Industrial •Debe tener licencia en Salud Ocupacional

2. CAPACITACIÓN Y ENTRENAMIENTO: El CONTRATISTA es responsable de asegurar que el personal a su cargo esté calificado para desarrollar con seguridad el trabajo contratado, incluyendo las metodologías utilizadas para analizar los riesgos inherentes a los trabajos a ejecutar y definir las medidas de seguridad que se deben observar en la ejecución de los mismos, y en cómo reaccionar frente a situaciones de emergencia. La formación debe incluir riesgos potenciales conocidos de incendio, explosión o emisiones tóxicas, relacionadas con su trabajo y proceso; reglas, normas, estándares y demás instrucciones de Seguridad Industrial establecidas por CARVAJAL PULPA Y PAPEL Y CARVAJAL PULPA Y PAPEL SAS ZONA FRANCA PERMANENTE ESPECIAL, relacionadas con las actividades a cargo del CONTRATISTA; elementos aplicables del plan de respuesta y control de emergencias, y prácticas de trabajo seguro aplicables. El personal del CONTRATISTA y Subcontratista debe contar con la acreditación y certificación de competencias para las especialidades que así lo requieran de acuerdo con la legislación colombiana vigente (soldadores, electricistas, armadores y certificadores de andamios, operadores de equipos pesados, mecánicos, conductores, trabajo en alturas, etc.); Las certificaciones deben ser expedidas por instituciones avaladas nacional o internacionalmente, de conformidad con la legislación colombiana. El personal debe ser entrenado en los procedimientos críticos que le conciernen, competente, apto e idóneo en su tarea CARVAJAL PULPA Y PAPEL Y CARVAJAL PULPA Y PAPEL SAS ZONA FRANCA PERMANENTE ESPECIAL podrá solicitar sustitución de personal cuando el CONTRATISTA no acredite que tiene las competencias para ejecutar los trabajos que le fueron asignados o no cumpla con los procedimientos, dado que ello, además de implicar un incumplimiento del Contrato, conlleva riesgos para la vida propia, de los demás, para el medio ambiente y/o infraestructura y bienes de la empresa. Cuando por razón del alcance del Contrato se deban realizar actividades tales como construcción, interventoría, recepción, operación, mantenimiento o inspección, que conlleven intervención de una instalación eléctrica o de un sistema eléctrico, estas deberán ser realizadas por personas calificadas, con matricula profesional, certificado de inscripción profesional o certificado de matrícula, que de acuerdo a la legislación vigente lo faculte para ejercer dicha actividad. Tales personas responderán por los efectos resultantes de su participación en la instalación. La competencia para realizar dichas actividades corresponderá a las personas calificadas, tales como: ingenieros electricistas, electromecánicos, de distribución y redes eléctricas, o electrónicos en los temas de electrónica de potencia, control o compatibilidad electromagnética; tecnólogos en electricidad, tecnólogos en electromecánica o técnicos electricistas con matrícula profesional, certificado de inscripción profesional o certificado de matrícula, vigentes, teniendo en cuenta lo dispuesto en las leyes y normas reglamentarias que regulan el ejercicio de cada una de estas profesiones, en especial lo referido a sus competencias.

3. CUMPLIMIENTO LEGAL: El CONTRATISTA, durante toda la ejecución del Contrato, debe contar con una matriz de identificación de los requisitos legales en Seguridad Industrial, Salud Ocupacional y Medio Ambiente aplicables al objeto del mismo, establecer los mecanismos para el cumplimiento de tales requisitos; y mantener la información actualizada. Debe comunicar la información pertinente sobre requisitos legales y de otra índole a todos sus empleados y demás partes interesadas.

4. INDUCCIÓN Y REINDUCCIÓN: El CONTRATISTA debe asistir a la Inducción de Seguridad y medio Ambiente programa por CARVAJAL PULPA Y PAPEL Y CARVAJAL PULPA Y PAPEL SAS ZONA FRANCA PERMANENTE ESPECIAL antes de iniciar la labor, esta inducción tiene una vigencia de 1 año y debe ser renovada antes de su vencimiento.

5. PROCEDIMIENTOS Y PRÁCTICAS SEGURAS: El CONTRATISTA debe contar con procedimientos y practicas seguras para las actividades y tareas críticas objeto del Contrato, los cuales deben estar redactados de forma coherente y clara, y estar disponibles para todo el personal. El CONTRATISTA debe asegurarse que sus trabajadores y Subcontratistas, los conozcan y ejecuten con rigurosidad, tomando acciones frente a las desviaciones. El CONTRATISTA debe conocer y acatar los procedimientos y prácticas seguras establecidas por CARVAJAL PULPA Y PAPEL Y CARVAJAL PULPA Y PAPEL SAS ZONA FRANCA PERMANENTE ESPECIAL, cuando se realicen trabajos dentro de las instalaciones de la empresa. Dentro de ellos se tienen: Si el Contrato contempla trabajos de alto riesgo y/o actividades de reparación, modificación, construcción, instalación, inspección y prueba de equipos o componentes críticos para la seguridad de los procesos, como mínimo se debe: a) Entrada a Espacio Confinado con Atmósfera Peligrosa: Disponer de 1 Centinela: Persona ubicada en el exterior, designada para observar y ayudar al trabajador que está en el interior del espacio confinado, Herramientas de materiales que no produzcan chispas (lámpara antichispa y herramientas neumática, Arnés de seguridad, cuerdas de salvamento y redes de seguridad, Equipo de ventilación (ventiladores o equipos tipo Venturi), Elementos de protección personal de acuerdo al riesgo y todas las demás exigencias del permiso de trabajo que debe ser autorizado por el operador del área. Cumplir todo lo exigido en la Norma No, 991450049 b) Apertura de Líneas de procesos y equipos: Se debe cumplir con todas las exigencias establecidas en la norma interna No. 991450057. Entre las principales, se debe usar traje completo para químicos, drenajes, eliminación de emergencia residual y estado de energice. Diligenciar el formato Ni Riesgos c) Trabajos en caliente: Certificación de competencia de soldador, disponer de 1 Centinela por trabajo: Persona designada para vigilar que no se produzca un incendio durante la ejecución del trabajo y revisar el área durante 1 hora después de concluido este. Equipos: Un cilindro de oxigeno o de gas propano, Un cilindro de acetileno, Un juego de mangueras, Un mango mezclador y un mango cortador, Regulador de oxígeno y de acetileno con sus respectivos manómetros, Válvulas arresta llamas, Maquina de soldar eléctrica, Cable para la masa y cable para la pinza, Cable para la clavija, Extintor multipropósito. Y lo demás exigido en la norma No. 991450050. d) Proveedores que manipulen fuentes / sustancias radioactivas y Radiografías Industriales: Tener acreditación para por Ingeominas, Notificación al Departamento de Seguridad Industrial/Salud ocupacional y Gestión Ambiental de la fecha y sitio donde se va a realizar la radiografía, Notificación sobre el área de la Planta en la cual se va a realizar el trabajo, usar contador Geiger calibrado, acordonar (aislar) las áreas donde se realiza la radiografía industrial, tener un plan de respuesta a emergencia por radiaciones ionizantes. e) Trabajos en Altura: Se requiere certificación por entidad competente en Colombia con vigencia de reentrenamiento menor a un año de acuerdo a lo exigido por la legislación en Colombia, diligenciar y cumplir con todas las exigencias del permiso de trabajo, uso de sistemas de acceso (andamios plataformas, elevadores de personas, andamios, y demás) certificados, equipos de protección contra caídas (arnés, eslingas, conectores, líneas de vida, entre otros) certificados. Se debe garantizar la supervisión permanente del área con un ayudante de seguridad. Se debe dar cumplimiento de la Resolución 1409 de 2012. f) Excavaciones: Diligenciar formato Ni Riesgos. g) Manejo de Cargas: Izaje con Grúas: Se debe tener certificación tanto de la persona que opera (Operador de grúa y aparejador) como del equipo (grúa) de acuerdo a disponer de un plan de izaje para realizar el trabajo. h) Trabajos Eléctricos: El personal electricista deberá tener matrícula profesional con la categoría, que según la normatividad legal vigente, lo autorice o acredite para el ejercicio de la profesión en la labor contratada. i) Transporte y manipulación de productos químicos peligrosos: cumplir con Decreto 1609/2002 para transporte de mercancías peligrosas y toda la legislación colombiana relacionada con transporte y manipulación de mercancías peligrosas. Para todos los trabajos se debe diligenciar y ser aprobado por el operador del área el Formato Ni Riesgos entregado por CARVAJAL PULPA Y PAPEL Y CARVAJAL PULPA Y PAPEL SAS ZONA FRANCA PERMANENTE ESPECIAL en la enfermería cada planta, el CONTRATISTA debe velar porque los controles identificados en este análisis de riesgos se implementen. Avisar a CARVAJAL PULPA Y PAPEL Y CARVAJAL PULPA Y PAPEL SAS ZONA FRANCA PERMANENTE ESPECIAL la presencia de riesgos propios del trabajo del CONTRATISTA no identificados inicialmente, o riesgos encontrados durante el desarrollo del trabajo del CONTRATISTA.

6. IDENTIFICACION ASPECTOS / IMPACTOS – PELIGROS / RIESGOS: El CONTRATISTA debe alimentar su matriz de identificación de peligros y riesgos / aspectos e impactos ambientales propios de su actividad, con los riesgos específicos del lugar donde se realizará el trabajo. Dicha matriz deberá ser actualizada como mínimo anualmente, además deberán ser implementadas las medidas de control requeridas para la administración de los riesgos.

7. ELEMENTOS DE PROTECCIÓN PERSONAL: El CONTRATSITA debe entregar dotación de elementos de protección personal de acuerdo al riesgo y garantizar certificación de acuerdo a normas nacionales o internacionales vigentes

8. MEDICINA PREVENTIVA Y DEL TRABAJO: El CONTRATISTA debe establecer el alcance de los exámenes de ingreso, periódicos, post incapacidad y de egreso, requeridos por todo el personal, de acuerdo con las funciones y el cargo a desempeñar. Debe contar con los certificados de aptitud de cada trabajador emitido por Médico Especialista en Salud Ocupacional, acorde a la normativa (Resolución 2346 de 2007). Para trabajos de alto riesgo, se debe especificar en el certificado que es apto para el trabajo crítico respectivo, de conformidad con la normatividad legal vigente. El CONTRATISTA deberá disponer de esta información por cuanto CARVAJAL PULPA Y PAPEL Y CARVAJAL PULPA Y PAPEL SAS ZONA FRANCA PERMANENTE ESPECIAL la podrá solicitar en cualquier momento, a través de Seguridad Industrial y Salud Ocupacional. Debe incluir dentro de su programa actividades campañas específicas, tendientes a fomentar la prevención y el control de la fármaco dependencia, el alcoholismo y el tabaquismo, dirigidas a sus trabajadores. El CONTRATISTA implementará las Actividades de Promoción y Protección de la Salud, requeridas según el Diagnóstico en Salud y los Riesgos para la Salud propios de cada trabajo, ejecutando los Programas de Vigilancia Epidemiológica, Inmunizaciones, Auto cuidado, Estilos de Vida Saludable, control del estrés laboral, riesgo psicosocial y otras enfermedades.

9. HIGIENE INDUSTRIAL: Se deben revisar los estudios de higiene industrial de las áreas donde se ejecutara el Contrato, con el fin de conocer los niveles de ruido, vibraciones, iluminación, los agentes químicos presentes en las actividades y del medio en el cual se realizan los trabajos etc., según aplique; así como una posterior comparación contra los límites máximos permisibles de acuerdo a la legislación colombiana vigente; con el fin de incorporarlos en sus análisis y controles de riesgos. Para la medición de los agentes químicos presentes en el medio de trabajo y de interés ocupacional, en caso de requerirse, se debe contar con equipos portátiles o personales, calibrados por una persona competente, o monitoreo realizado por Proveedor competente, con Licencia en Salud Ocupacional vigente y certificaciones de calibración de equipos vigentes.

10. PLAN DE RESPUESTA A EMERGENCIAS: El contratista debe tener un plan de respuesta a emergencias de acuerdo a lo exigido por la legislación colombiana.

11. SUBCONTRATISTAS: El contratista debe exigir el mismo requerimiento de este documento cuando realice contratación de terceros para realizar un trabajo dentro de las instalaciones de CARVAJAL PULPA Y PAPEL Y CARVAJAL PULPA Y PAPEL SAS ZONA FRANCA PERMANENTE ESPECIAL.

12. VEHÍCULOS DENTRO DE LA COMPAÑÍA: El contratista que requiera ingresar vehículos propios o contratados para ejecutar la labor de su contrato dentro de las instalaciones de la compañía debe cumplir todos los requerimientos de ley en cuanto revisión técnico mecánica, revisión de gases, licencias de conducción y demás requisitos exigidos por la legislación colombiana vigente y lo exigido por la Resolución 1565 del Plan estratégico de seguridad vial

13. PAGO DE SEGURIDAD SOCIAL: En caso de ser un trabajador recién vinculado a la empresa contratista, debe aportar copia de la afiliación a EPS, AFP, Caja de Compensación y ARL donde se evidencie el nivel de riesgo, radicadas en cada una de las entidades. En caso de estar vinculado a la empresa contratista con anterioridad, la planilla de pago a la seguridad social, donde se evidencie el nivel de riesgo y certificación no inferior a un día de afiliación vigente expedida por la ARL. En todos los casos la afiliación a estas entidades debe hacerse en cabeza de la empresa contratista, no podrá usarse sociedades, asociaciones, corporaciones, fundaciones y la contratación de servicios de colaboración o manejo de recurso humano, Compañías de Servicios de Colaboración, o personas naturales que no ejerzan como verdaderos empleadores de los colaboradores.

14. MEDIO AMBIENTE: El PROVEEDOR cumplirá con la normatividad ambiental aplicable a su actividad comercial, el Estudio de Impacto Ambiental, el Plan de Manejo Ambiental, Medidas de Manejo Ambiental y permisos, según apliquen. Mantendrá sus instalaciones y equipos en óptimas condiciones de orden y aseo, asegurando diariamente la gestión y evacuación de los residuos previamente separados y clasificados a los sitios de disposición final autorizados debidamente por las Autoridades Ambientales. Para ello, contará con un procedimiento de manejo y disposición de residuos domésticos e industriales, que refleje que la gestión integral de los residuos se realiza de acuerdo con la normatividad Nacional que aplique. El PROVEEDOR debe contar también con la MATRIZ LEGAL AMBIENTAL. El PROVEEDOR deberá entregar el plan de manejo ambiental, los permisos o comunicados donde certifican el sitio de extracción de material o canteras autorizadas por la CRC o CVC y la firma de la persona (natural o jurídica) que posee la licencia minero-ambiental otorgada por Ingeominas.

15. ASISTENCIA A ACTIVIDADES PROGRAMADAS POR SEGURIDAD, SALUD Y MEDIO AMBIENTE: Las empresas convocadas a actividades programadas por CARVAJAL PULPA Y PAPEL Y CARVAJAL PULPA Y PAPEL SAS ZONA FRANCA PERMANENTE ESPECIAL debe asistir obligatoriamente, la no asistencia genera aviso de calidad. Todo escombro debe ser retirado fuera de CARVAJAL PULPA Y PAPEL Y CARVAJAL PULPA Y PAPEL SAS ZONA FRANCA PERMANENTE ESPECIAL S.A en sitios autorizados por la CRC o la CVC y se debe entregar certificado de la escombrera autorizada en gestión ambiental de CARVAJAL PULPA Y PAPEL Y CARVAJAL PULPA Y PAPEL SAS ZONA FRANCA PERMANENTE ESPECIAL.

3.PARAGRAFO III: SUMINISTRO DE CERTIFICACIONES VIGENTES:

EL PROVEEDOR es el único responsable ante CARVAJAL PULPA Y PAPEL Y CARVAJAL PULPA Y PAPEL SAS ZONA FRANCA PERMANENTE ESPECIAL y ante las autoridades internacionales, nacionales y locales por la veracidad de las certificaciones presentadas de los sistemas de gestión. De igual forma se compromete mediante este documento a suministrar al área de abastecimiento de CPP las certificaciones actualizadas correspondientes a las exigidas en el momento de su inscripción, como condición para su permanencia como proveedor activo. En caso de no tener certificaciones en el momento de registro, EL PROVEEDOR se compromete a enviarlas cuando las obtenga, o en su defecto certificación equivalente otorgada por un tercero y reconocida por CPP, asumiendo la obligación de efectuar entregas con un porcentaje bajo de defectos y mejorar continuamente su desempeño.

4.PARAGRAFO IV: CONVENIO DE SEGURIDAD: POLITICA DE CONTROL Y SEGURIDAD:

 La política se constituye a partir de los elementos relacionados con el Control y la Seguridad que se encuentran en las políticas de CARVAJAL PULPA Y PAPEL Y CARVAJAL PULPA Y PAPEL SAS ZONA FRANCA PERMANENTE ESPECIAL y que tienen la finalidad de minimizar el riesgo en las exportaciones e importaciones respecto al contrabando, narcotráfico, terrorismo o cualquier otra actividad ilícita y además mantener un ambiente de seguridad en las instalaciones y la comunidad. Parte del cumplimiento a esta política está representada en los acuerdos de seguridad con EL PROVEEDOR, a través de la integración y participación activa de procedimientos y políticas claras para que la relación comercial esté basada en operaciones seguras, confiables y trazables.

4.1. POR PARTE DE CARVAJAL PULPA Y PAPEL Y CARVAJAL PULPA Y PAPEL SAS ZONA FRANCA PERMANENTE ESPECIAL, Y/O SUBSIDIARIAS: CARVAJAL PULPA Y PAPEL Y CARVAJAL PULPA Y PAPEL SAS ZONA FRANCA PERMANENTE ESPECIAL dará cumplimiento a los requisitos aplicables a su sistema de Gestión en Control y Seguridad. Con esto, se busca asegurar que el sistema de Gestión en Control y Seguridad esté apoyado por EL PROVEEDOR, y que puede confiar en que de igual forma EL PROVEEDOR tomará las medidas necesarias y pertinentes, para garantizar que la mercancía transportada por terceros será controlada de forma tal que se evite cometer cualquier acto ilícito por intermedio suyo, convirtiéndose así en un asociado de negocio para garantizar una cadena de comercio seguro.

4.2. BUENAS PRÁCTICAS DE SEGURIDAD A CARGO DEL PROVEEDOR: 4.2.1. Mantener como prioridad permanente el monitoreo y la medición de los incidentes que puedan poner en riesgo las operaciones. 4.2.2.Tener procedimientos para la selección y evaluación de sus proveedores. 4.2.3.Inspeccionar los contenedores antes del cargue, se recomienda una inspección de siete puntos: Pared delantera, lado izquierdo, lado derecho, piso, techo interior/exterior, puertas interiores/exteriores, exterior / sección inferior. Además de realizar inspecciones a los vehículos y tráileres que transportan la mercancía. (Si aplica). 4.2.4.Garantizar que la integridad de los contenedores, remolques y mercancías sean protegidos contra la introducción de materiales y/o personas no autorizados (especialmente en el punto de llenado) y utilizar los sellos y precintos de acuerdo a las normas de seguridad vigente. (Si aplica). 4.2.5. El almacenamiento de los sellos y precintos se hará en lugares seguros y bajo llave, su uso será controlado por el personal designado para ello. Los sellos no serán usados en estricto orden de numeración. De igual forma deberá tener procedimientos para reportar precintos dañados o alterados. (Si aplica). 4.2.6. Adoptar medidas de seguridad para garantizar la integridad y seguridad de los procesos relevantes al transporte, manejo y almacenaje de la carga en la cadena de suministro. 4.2.7. Garantizar que toda la información y documentación utilizada para despachar mercancías sea legible, completa y exacta. 4.2.8. Describir con exactitud las características de las materias primas o productos, indicando el peso, unidad de empaque, etiquetas, marcas; así mismo realizará un conteo de las piezas o unidades frente a los documentos de la carga. La carga que se está enviando deberá compararse con las órdenes de compra o de entrega. Así mismo, identificará positivamente a los conductores antes de que reciban o entreguen la carga. 4.2.9. Mantener protocolos o procedimientos para reportar oportunamente a las autoridades en caso de detectar anomalías o discrepancias con la mercancía, o actividades ilegales o sospechosas. 4.2.10. Disponer de procedimientos para la selección del personal, el cual debe incluir la verificación de referencias, de antecedentes y realización de visitas domiciliarias. Estos acuerdos entre EL PROVEEDOR y CARVAJAL PULPA Y PAPEL Y CARVAJAL PULPA Y PAPEL SAS ZONA FRANCA PERMANENTE ESPECIAL podrán ser verificados por ambas partes en el momento en que se considere necesario y serán evaluados cuando situaciones extraordinarias se presenten. Además, servirán como parámetro evaluador dentro de los sistemas de gestión como proveedores confiables y serán retroalimentados de manera oportuna.

5. PARAGRAFO V: CUMPLIMIENTO DE REQUISITOS MINIMOS DE SEGURIDAD EN LA CADENA DE SUMINISTRO INTERNACIONAL:

6. 8. PREVENTIVE AND WORK MEDICINE: The CONTRACTOR must establish the scope of the entrance exams, periodicals, post disability and discharge, required by all personnel, according to the functions and the position to be performed. Must have the certificates of competence of each worker issued by a Specialist in Occupational Health, according to the regulations (Resolution 2346 of 2007). For high-risk jobs, it must be specified in the certificate that is suitable for the respective critical work, in accordance with current legal regulations. The CONTRACTOR must have this information as CARVAJAL PULP AND PAPER AND CARVAJAL PULP AND PAPER SAS SPECIAL FREE TRADE ZONE may request it at any time, through Industrial Safety and Occupational Health. It must include specific campaigns within its program, aimed at promoting the prevention and control of drug dependency, alcoholism and smoking, aimed at its workers. The CONTRACTOR will implement the Health Promotion and Protection Activities, required according to the Health Diagnosis and the Health Risks of each job, executing the Epidemiological Surveillance Programs, Immunizations, Self-care, Healthy Lifestyles, stress control labor, psychosocial risk and other diseases.

9. INDUSTRIAL HYGIENE: The industrial hygiene studies of the areas where the Contract will be executed should be reviewed, in order to know the levels of noise, vibrations, lighting, the chemical agents present in the activities and the environment in which perform the work etc., as applicable; as well as a later comparison against the maximum permissible limits according to current Colombian legislation; in order to incorporate them into their risk analysis and controls. For the measurement of chemical agents present in the work environment and of occupational interest, if required, portable or personal equipment, calibrated by a competent person, or monitoring performed by a competent Supplier, with Occupational Health License, must be available. in force and current equipment calibration certifications.

10. EMERGENCY RESPONSE PLAN: The contractor must have an emergency response plan according to what is required by Colombian legislation.

11. SUBCONTRACTORS: The contractor must demand the same requirement of this document when hiring third parties to perform work within the facilities of CARVAJAL PULP AND PAPER AND CARVAJAL PULP AND PAPER SAS SPECIAL FREE TRADE ZONE.

12. VEHICLES WITHIN THE COMPANY: The contractor that requires to enter their own or contracted vehicles to perform the work of their contract within the company's facilities must comply with all the requirements of the law regarding mechanical technical revision, gas revision, license driving and other requirements required by current Colombian legislation and as required by Resolution 1565 of the Strategic Road Safety Plan

13. SOCIAL SECURITY PAYMENT: In case of being a worker newly linked to the contractor company, you must provide a copy of the affiliation to EPS, AFP, Compensation Fund and ARL where the risk level is evidenced, located in each of the entities. In case of being linked to the contractor beforehand, the payment form to the social security, where the level of risk and certification is evidenced not less than one day of current membership issued by the ARL. In all cases the affiliation to these entities must be made at the head of the contractor company, companies, associations, corporations, foundations and the hiring of collaboration services or human resource management, Collaboration Service Companies, or individuals may not be used. that they do not practice as true employers of the collaborators.

14. ENVIRONMENT: The SUPPLIER will comply with the environmental regulations applicable to its commercial activity, the Environmental Impact Study, the Environmental Management Plan, Environmental Management Measures and permits, as applicable. It will maintain its facilities and equipment in optimal conditions of order and cleanliness, ensuring daily the management and evacuation of previously separated and classified waste to the disposal sites duly authorized by the Environmental Authorities. For this, it will have a procedure for the handling and disposal of domestic and industrial waste, which reflects that the integral management of the waste is carried out in accordance with the applicable National regulations. The SUPPLIER must also have the LEGAL ENVIRONMENTAL MATRIX. The SUPPLIER must deliver the environmental management plan, the permits or releases certifying the site for the extraction of material or quarries authorized by the CRC or CVC and the signature of the person (natural or legal) that holds the mining-environmental license granted by Ingeomins.

15. ASSISTANCE TO ACTIVITIES SCHEDULED BY SECURITY, HEALTH AND ENVIRONMENT: The companies called to activities programmed by CARVAJAL PULP AND PAPER AND CARVAJAL PULP AND PAPER SAS SPECIAL FREE TRADE ZONE must attend, the non-attendance generates quality notice. 3.PARAGRAPH III: SUPPLY OF CURRENT CERTIFICATIONS:

4.PARAGRAPH IV: SECURITY AGREEMENT:CONTROL AND SECURITY POLICY

The policy is based on the elements related to Control and Security found in the policies of CARVAJAL PULP AND PAPER AND CARVAJAL PULP AND PAPER SAS SPECIAL FREE TRADE ZONE and which are intended to minimize the risk in exports and imports with respect to smuggling, drug trafficking, terrorism or any other illicit activity and also maintain a safe environment in the facilities and the community. Part of the compliance with this policy is represented in the security agreements with THE supplier, through the integration and active participation of clear procedures and policies so that the commercial relationship is based on safe, reliable and traceable operations. 

4.1. BY CARVAJAL PULP AND PAPER AND CARVAJAL PULP AND PAPER SAS SPECIAL FREE TRADE ZONE, AND / OR SUBSIDIARIES: CARVAJAL PULP AND PAPER AND CARVAJAL PULP AND PAPER SAS SPECIAL FREE TRADE ZONE will comply with the requirements applicable to its Management in Control system and Security. 4.2. GOOD SECURITY PRACTICES BY THE supplier: 4.2.1. Maintain as a permanent priority the monitoring and measurement of incidents that may put operations at risk. 4.2.2. Have procedures for the selection and evaluation of their suppliers. 4.2.3.Inspect the containers before loading, a seven-point inspection is recommended: Front wall, left side, right side, floor, interior / exterior roof, interior / exterior doors, exterior / lower section. In addition to carrying out inspections of the vehicles and trailers that transport the merchandise. (If applicable) 4.2.4.To ensure that the integrity of containers, trailers and merchandise are protected against the introduction of unauthorized materials and / or persons (especially at the filling point) and to use seals and seals in accordance with current safety regulations. . (If applicable) 4.2.5. The storage of seals and seals will be done in secure places and under lock and key, its use will be controlled by the personnel designated for it. The stamps will not be used in strict order of numbering. In the same way, you must have procedures to report damaged or altered seals. (If applicable) 4.2.6. Adopt safety measures to ensure the integrity and safety of the processes relevant to the transport, handling and storage of cargo in the supply chain. 4.2.7. Ensure that all information and documentation used to clear goods is legible, complete and accurate. 4.2.8. Describe exactly the characteristics of the raw materials or products, indicating the weight, packaging unit, labels, brands; Likewise, it will make a count of the pieces or units in front of the documents of the load. The load that is being shipped should be compared with the purchase or delivery orders. Likewise, it will positively identify the drivers before they receive or deliver the cargo. 4.2.9. Maintain protocols or procedures to timely report to the authorities in case of detecting anomalies or discrepancies with the merchandise, or illegal or suspicious activities. 4.2.10. Have procedures for the selection of personnel, which should include verification of references, background and home visits. These agreements between THE supplier and CARVAJAL PULPA Y PAPEL AND CARVAJAL PULPA AND PAPEL SAS SPECIAL PERMANENT FREE ZONE may be verified by both parties at the time deemed necessary and will be evaluated when extraordinary situations arise. 5. PARAGRAPH V: COMPLIANCE WITH MINIMUM SAFETY REQUIREMENTS IN THE INTERNATIONAL SUPPLY CHAIN:

The SUPPLIER accepts that all the goods and services provided come from legal activities related to their economic activity, in the case of being imported materials they have been duly nationalized and have all the documentation required by Colombian legislation. The SUPPLIER guarantees contingency plan for the supply / well contracted.
